Who operates this website

Mave Insurance, LLC operates this website. Contact Hello@maveinsurance.com, call 407-800-2913, or write to 7726 Winegard Rd., Suite 200, Orlando, FL 32809 with a privacy question. This notice describes the website; an insurer or servicing agency may provide additional insurance-specific privacy notices when required.

Information you choose to provide

An inquiry can include your name, email, optional phone, state, ZIP code, coverage interest, broad timing, language, contact preference and a short description of your goal. An agent introduction can also include licensing stage, self-reported licensed states, product focus and career goals. We also record your contact acknowledgment, a generated request reference and submission time. Do not provide Social Security numbers, full birth dates, Medicare IDs, diagnoses, medical records, policy numbers or payment details.

How an inquiry is handled

The form securely transmits the inquiry through Formspree, Mave’s form-delivery provider, for delivery to inquiry@maveinsurance.com so Mave can respond and identify an appropriate next step. Formspree processes the submitted fields to provide this delivery service. The inquiry does not create a CRM contact, submit a carrier application or enroll you. A private preview explicitly identifies when delivery is not connected and does not transmit or save the entered inquiry.

Medicare e-book and life intake

The Medicare e-book form requires name, email and phone and separately records a Yes or No choice for agent contact. No still allows the download. A No request is explicitly marked Do not contact and Do not call in the submission and notification. Mave must honor that preference in any later manual processing; this website does not connect requests to a dialer or call campaign. Yes requests an individual follow-up about Medicare questions, not automated calls, marketing texts or recurring campaigns. A later request to stop is marked Do not call and further outreach must stop. The life intake also collects broad goals, desired coverage, duration, existing-coverage category and optional budget preference. Neither form requests medical information or submits a carrier application.

Appointment booking

Enrollment preparation links open Calendly, which manages booking details, availability, confirmation and reminders under its own privacy practices. Your contact details are not automatically included in booking URLs. Choosing not to receive agent outreach on an e-book request does not book an appointment. A booking is not Medicare enrollment or a Scope of Appointment; any required additional steps are handled separately.

Sharing and partner permissions

Mave may use hosting, form-delivery, email and other necessary service providers to operate the website and handle requests. Mave assesses each coverage inquiry around the person’s specific wants and needs and coordinates a response from Mave or one suitable affiliated partner. Requests are not broadcast to every agency. Before an outside agency receives information or contacts you, Mave identifies the recipient and obtains permission where required. Medicare beneficiary data sharing can require separate, entity-specific written consent. Information may also be disclosed when legally required or necessary to protect lawful rights and website security.

Contact choices

The inquiry acknowledgment requests a response from Mave using your selected contact method. It does not subscribe you to automated marketing calls, texts or an advertising list, and it is not a condition of purchasing insurance. You can ask Mave to stop responding or update your contact preference by using the contact information above.

Cookies, analytics and technical records

Essential sessions help protect forms. Device-local storage can remember cookie choices, accessibility preferences, enrollment announcement frequency and checklist progress. Optional analytics runs only when configured and you allow it; it is disabled in the private preview. The analytics implementation excludes inquiry values, names, email addresses, phone numbers and URL query strings. Hosting and email providers may maintain operational records. Short-lived security records may contain hashed technical identifiers and request references, but the website does not store the inquiry body in a customer database.

Retention and requests

Email correspondence is retained as reasonably needed to respond, service an established relationship, meet legal recordkeeping duties, resolve disputes and protect security. Retention requirements depend on the record and applicable rules. Ask us about access, correction, deletion or contact preferences; we may need to verify identity and retain information where law requires. Do not send identity documents with an initial privacy request.

Security, children and outside sites

We use reasonable safeguards, but no website or email system can promise absolute security. Use approved secure insurer or agency channels for applications and sensitive records. This website is intended for adults and is not directed to children under 13. External partner, insurer and government sites have their own privacy practices. Material changes to this notice will be posted here with an updated date.
